How It Works
The pattern names an interpretation, negates it for the sake of argument, then asks what alternative remains. یہ مدد نہیں تو اور کیا ہے؟ strongly presents the act as help.
The opening can contain نہیں or a fuller conditional form with نہ. The entire construction matters more than a word-for-word translation of the negative.
The question can be playful, appreciative, or argumentative. It expresses the speaker’s stance; it is not proof that no other interpretation is possible.

The long subject is an action being interpreted as friendship.
